VA Disability Rating for PTSD
Diagnostic code 9411, posttraumatic stress disorder, is rated under the General Rating Formula for Mental Disorders in 38 CFR 4.130. The same formula covers the other mental-disorder codes from 9201 through 9440.
Current General Rating Formula
The regulation lists symptoms “such as” the ones below. It does not say every symptom must be present. The occupational and social impairment description is part of each level.
| Rating | Criteria | 2026 monthly, veteran alone |
|---|---|---|
| 100% | Total occupational and social impairment, due to such symptoms as: gross impairment in thought processes or communication; persistent delusions or hallucinations; grossly inappropriate behavior; persistent danger of hurting self or others; intermittent inability to perform activities of daily living (including maintenance of minimal personal hygiene); disorientation to time or place; memory loss for names of close relatives, own occupation, or own name. | $3,938.58 |
| 70% | Occupational and social impairment, with deficiencies in most areas, such as work, school, family relations, judgment, thinking, or mood, due to such symptoms as: suicidal ideation; obsessional rituals which interfere with routine activities; speech intermittently illogical, obscure, or irrelevant; near-continuous panic or depression affecting the ability to function independently, appropriately and effectively; impaired impulse control (such as unprovoked irritability with periods of violence); spatial disorientation; neglect of personal appearance and hygiene; difficulty in adapting to stressful circumstances (including work or a worklike setting); inability to establish and maintain effective relationships. | $1,808.45 |
| 50% | Occupational and social impairment with reduced reliability and productivity due to such symptoms as: flattened affect; circumstantial, circumlocutory, or stereotyped speech; panic attacks more than once a week; difficulty in understanding complex commands; impairment of short- and long-term memory (e.g., retention of only highly learned material, forgetting to complete tasks); impaired judgment; impaired abstract thinking; disturbances of motivation and mood; difficulty in establishing and maintaining effective work and social relationships. | $1,132.90 |
| 30% | Occupational and social impairment with occasional decrease in work efficiency and intermittent periods of inability to perform occupational tasks (although generally functioning satisfactorily, with routine behavior, self-care, and conversation normal), due to such symptoms as: depressed mood, anxiety, suspiciousness, panic attacks (weekly or less often), chronic sleep impairment, mild memory loss (such as forgetting names, directions, recent events). | $552.47 |
| 10% | Occupational and social impairment due to mild or transient symptoms which decrease work efficiency and ability to perform occupational tasks only during periods of significant stress, or symptoms controlled by continuous medication. | $180.42 |
| 0% | A mental condition has been formally diagnosed, but symptoms are not severe enough either to interfere with occupational and social functioning or to require continuous medication. | $0.00 |
A 0% rating has no amount in VA’s compensable tables, which start at 10%. Dependent amounts start at 30%. See the 2026 pay chart.

Note 2 in 38 CFR 4.130: ratings under diagnostic codes 9201 to 9440 will be evaluated using the General Rating Formula for Mental Disorders. The eCFR citation for this section is 79 FR 45100, Aug. 4, 2014. Authority: 38 USC 1155.
A 2022 proposal is not the current formula
On February 15, 2022, VA published a proposed rule, 87 FR 8498 (FR Doc. 2022-02051, RIN 2900-AQ82), to revise the portion of the rating schedule dealing with mental disorders, including the General Rating Formula for Mental Disorders. That document is a proposed rule. It does not set the ratings in the table above.
A Federal Register API search on October 20, 2026 for “2900-AQ82” returned that proposed rule and an August 14, 2026 Unified Agenda notice (document 2026-16603). It did not return a final rule. The eCFR issue dated August 27, 2026 still uses the formula quoted here.
