VA Disability Rating for IBS
Irritable bowel syndrome is diagnostic code 7319 in 38 CFR 4.114. The criteria below are the current schedule. VA revised this portion of the rating schedule effective May 19, 2024.
Current rating criteria (DC 7319)
| Rating | Criteria in 38 CFR 4.114 | 2026 monthly, veteran alone |
|---|---|---|
| 30% | Abdominal pain related to defecation at least one day per week during the previous three months; and two or more of the following: (1) change in stool frequency, (2) change in stool form, (3) altered stool passage (straining and/or urgency), (4) mucorrhea, (5) abdominal bloating, or (6) subjective distension | $552.47 |
| 20% | Abdominal pain related to defecation for at least three days per month during the previous three months; and two or more of the following: (1) change in stool frequency, (2) change in stool form, (3) altered stool passage (straining and/or urgency), (4) mucorrhea, (5) abdominal bloating, or (6) subjective distension | $356.66 |
| 10% | Abdominal pain related to defecation at least once during the previous three months; and two or more of the following: (1) change in stool frequency, (2) change in stool form, (3) altered stool passage (straining and/or urgency), (4) mucorrhea, (5) abdominal bloating, or (6) subjective distension | $180.42 |
The current code does not list a 0% level. At 10% and 20%, VA pays no extra for dependents. At 30%, dependents can change the amount. See the 2026 pay chart and the 50% page for how dependent additions work once a combined rating is 30% or higher.
Open the calculator with 30% filled in, 20%, or 10%. Other service-connected disabilities are combined under VA math, not added.
Note in DC 7319: This diagnostic code may include functional digestive disorders (see § 3.317), such as dyspepsia, functional bloating and constipation, and diarrhea. Evaluate other symptoms of a functional digestive disorder not encompassed by this diagnostic code under the appropriate diagnostic code, including gastrointestinal dysmotility syndrome (DC 7356), following the general principles of § 4.14 and this section. This page does not restate § 3.317 or DC 7356.
The May 19, 2024 digestive revision
VA published a final rule on March 20, 2024, 89 FR 19735 (FR Doc. 2024-05138). The rule says it is effective May 19, 2024. The amendatory history in that document lists DC 7319 as “title, evaluation, criterion, and note May 19, 2024.” The eCFR citation for 38 CFR 4.114 ends with 89 FR 19743, March 20, 2024. The table above is that current text. It is not a description of the criteria VA used before the effective date.
The opening sentence of 4.114 says not to combine ratings under diagnostic codes 7301 through 7329 inclusive, 7331, 7342, 7345 through 7350 inclusive, 7352, and 7355 through 7357 inclusive with each other. Assign a single evaluation under the diagnostic code that reflects the predominant disability picture, and elevate it to the next higher evaluation if warranted by the severity of the overall disability. DC 7319 is inside 7301–7329. GERD (DC 7206) is not in that list. 38 CFR 4.14 still says to avoid evaluating the same manifestation under different diagnoses.

Frequently asked questions
What ratings does diagnostic code 7319 list?
The current code lists 10%, 20%, and 30%. It does not list a 0% level. Each level requires abdominal pain related to defecation, on the schedule in the table, plus two or more of the listed bowel symptoms.
Did the IBS rating criteria change in 2024?
Yes. A final rule published March 20, 2024 (89 FR 19735) revised the digestive rating schedule effective May 19, 2024, including the title, evaluation, criterion, and note for DC 7319. The table on this page is the current text in 38 CFR 4.114, not the pre-2024 criteria.
Can IBS be combined with another digestive rating?
38 CFR 4.114 says not to combine ratings under diagnostic codes 7301 through 7329 inclusive, 7331, 7342, 7345 through 7350 inclusive, 7352, and 7355 through 7357 inclusive with each other. DC 7319 is in that list. When more than one of those ratings is warranted, VA assigns a single evaluation under the code that reflects the predominant disability, and elevates it to the next higher evaluation if the overall disability warrants it.
